Bree Dellamarta, 41, is alleged to have killed 55-year-old Mark Ward at Heidelberg West early on Friday.
Dellamarta has an intellectual disability, which her lawyer Jack Rabl says makes her vulnerable in custody.
He also told Melbourne Magistrates Court on Friday afternoon his client required anti-depressants, medication for asthma and an urgent medical assessment.
Dellamarta has never been in custody before and she sat in the dock with her arms crossed.
She did not apply for bail and is due back in court for a committal mention on May 28.
