NSW Police have revealed the occupants of the car, which had hit a tree, were a man aged in his 50s and three boys aged six, nine and 11 years old.
The man and two of the boys were airlifted from the scene to hospitals in Sydney.
The third boy was conveyed to hospital by road.
Police said the man and the three children are listed as being in a serious, but stable condition.
Officers from Chifley Police District have established a crime scene and are awaiting the arrival of specialist police.
Jenolan Caves Road is currently closed between the Great Western Highway and Lowther Siding Road.
Traffic diversions are expected to remain in place for a considerable amount of time and motorists are urged to avoid the area.

FOUR people, including three children, are reported to be in either a serious or critical condition following a single-vehicle crash south-east of Bathurst.
Emergency services are currently on the scene of the crash, which occurred on Jenolan Caves Road in Good Forest at around 2.30pm on Tuesday, April 1, 2025.
It is understood the car crashed into a tree.
A spokesperson for NSW Ambulance said the vehicle had four occupants, believed to be a man in his 50s and three male children.
Multiple road ambulance crews are in attendance and three helicopters have been assigned to the scene.
The NSW Ambulance spokesperson said all of the patients "are described as serious or critical".
They said the adult male appeared to have sustained chest and head injuries.
The first of the paediatric patients sustained head injuries and possible chest and abdominal injuries.
The second paediatric patient also has head injuries and possible limb fractures, while the third paediatric patient sustained injuries to his chest and abdomen.
It is expected patients will be airlifted to hospital from the scene.

Jenolan Caves Road is closed in both directions.
Diversions are in place for southbound and northbound light vehicles utilising the Great Western Highway, Magpie Hollow Road and Hampton Road.
This route will take an extra hour of travel time.
The diversion for westbound and eastbound heavy vehicles utilises the Great Western Highway, Littlebourne Street, O'Connell Road, Oberon Street, and Duckmaloi Road.
Heavy vehicle drivers are advised to allow for an extra two hours of travel time.
